The tenerife disaster was the worst aviation accident in history, it was a runway collision with 2 747's KLM and Pan-Am it happened in March of 1977.

1. The Tenerife disaster was caused by miscommunication. The resonsibility of keeping aircraft 'out of conflict' through clear and accurate communication belongs to the airport air traffic and ground controllers.
